 “The mission of CRMC Hospice Auxiliary is to provide uncompensated hospice support to terminally ill patients and their families. Volunteer participation will achieve this goal through fundraisers, joint community projects and financial donations offered at the individual as well as corporate level. Our efforts are to enhance, educate and embrace the many lives we are privileged to serve.”

We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.

Hospice care

Hospice care is not a physical place but a concept of sensitive, compassionate care that can be provided anywhere, including a patient’s home, a board and care facility, a skilled nursing facility, or a hospital. The goal of hospice is to improve a patient’s quality of life during his or her last days by providing care based on the support, compassion and dignity they need and deserve.

 

  • Hospice care neither prolongs life nor hastens death.
  • A team of specially trained professionals, volunteers and family members provide hospice care and specialized medical care, including pain management..
  • Hospice care addresses the symptoms of a disease and places a special emphasis on controlling and minimizing a patient’s pain and discomfort.
  • Hospice care deals with the emotional, spiritual, and social impact of the disease on the patient and his or her family and friends.
  • CRMC uses a multidiscilpinary team of sensitive, experienced and compassionate healthcare professionals who work under the direction of the patient’s primary care physician and our medical director.

Who Can Refer A Patient to Hospice?

 Anyone has the right to choose their hospice agency. This is a patient’s choice to meet their own needs. palliative care

Services We Provide

Nursing, Rehabilitation Therapies, Psychological Counseling and Support, Patient and Family Education; Volunteer Support and Training; Dietary Counseling; Spiritual Support and Guidance; and Home Health Aide. In addition, we coordinate the ordering, delivery and setup of necessary medical equipment and supplies. Benefits of Hospice Care

 Hospice stresses quality rather than length of life. When a terminal illness must be faced, hospice will educate a patient and family about the disease process and provide symptom control with an emphasis on minimizing pain and discomfort. Hospice is about support, compassion and dignity. The patient and family are involved in making decisions about patient care. Hospice is also about providing services and programs that can help people live complete and rich lives in spite of loss and grief.

Eligibility Criteria

Hospice care is intended for patients with terminal illnesses who are no longer seeking curative treatment, but instead want pain and symptom management, comfort and support. To be eligible for hospice care, patients must have a terminal prognosis with a life expectancy of six months or less, as certified by the patient’s primary care physician and the hospice medical director.

Reimbursement

Hospice care is fully covered by Medicare and Medi-Cal. This includes the cost of medical equipment, medications and other care related to the patient's hospice diagnosis. Most private insurance and managed care plans also cover hospice services. Patients enrolled in Senior HMO plans may elect hospice care which is covered through Medicare while retaining their HMO plan.
